Hyperbaric oxygen (HBO) is effective for treatment of cerebral ischemia but its effectiveness is limited by its ability to reduce oxidative stress. This study was conducted to determine whether aldose reductase inhibition would enhance the neuroprotective effect of HBO. Gerbil models of 10% global brain ischemia were established. The animals were randomized into 4 groups: sham, ischemia, HBO, and HBO + aldose reductase inhibitor (ARI), respectively. Ischemia was induced by 4-vessel occlusion for 10 min. HBO was started 30 min after reperfusion and continued for 4 h. ARI was administered for 7 days before induction of ischemia. Brain water content (BWC) was significantly decreased in all experimental groups following reperfusion. In the ischemic group, BWC was significantly increased as compared to the sham group. HBO significantly reduced BWC in comparison to ischemia group. ARI alone did not show significant changes in BWC. ARI pre-treatment combined with HBO significantly reduced BWC in comparison to ischemia and HBO alone. In addition, ARI alone showed a significant reduction in infarct volume. The infarct volume in the HBO + ARI group was significantly reduced in comparison to the ischemia group and significantly increased in comparison to the HBO group. The present results showed that HBO therapy could reduce the infarct volume and BWC in ischemic gerbil brain. However, this reduction was improved by ARI.1. Field of the Invention
